These are the reasons that will form the basic list (for which there was broad but not total consensus):

1. Insufficient squamous epithelial cells
2. Cell detail obscured by inflammatory cells
3. Poorly preserved i.e. marked cytolysis
4. Cell detail obscured by excessive cell clumping
5. Prior endocervical abnormality but no endocervical cells seen
6. Request form states “cervix not seen”
7. Insufficient squamous epithelial cells possibly due to excess lubricant
8. Vial does not contain brush head
9. Entirely composed of endocervical cells
10. 360deg sweep (x 5) not carried out
11. Sample taken more than 6 weeks prior to processing
12. Sample leaked – insufficient fluid in pot to process
13. Fluid level insufficient (but no evidence of leakage)
14. Too much fluid in pot / fluid level too high
15. Heavily blood stained
16. Contamination by lubricant

The following were also requested – but will not be added unless there is an outcry / general agreement within the blog that they should be implemented. Our comments in brackets:

Sample taken by untrained sample taker (who would know this)

Unlabelled / incorrectly labelled sample (how does this make the sample inadequate?)

Broken slide (old reason not relevant to LBC samples)

No sample vial received (then why would it get entered into system)

No sample taker ID (nothing to do with inadequate sample)

Sample taken from woman outside national age range (again – not relevant to inadequate)

Vial date exceeded (same as item 11)

Unless clinically indicated a cytobrush specimen alone is unsuitable

Sample obscured by bacteria (single request..)

Repeat after treatment of infections (sounds like an action not a reason for inadequacy)

Predominantly inflammatory cells with insufficient squamous epithelial cells (alternative wording for item 2)

We will leave this post up for a few weeks to see if people wish to comment / disagree etc. Thereafter we will make the change based on the basic list unless additional reasons force their way in.

Labs around the country have been reporting increases of anything from 40% to 100% (Stafford / Liverpool / South Wales) and sometimes even more. This increased workload has placed a huge strain on staff and has inevitably affected turnaround times at many places.

The surge in demand seems to have peaked though and most places seem to be heading back towards more normal workloads.

The categories of most interest would appear to be Borderline / Borderline (cannot exclude High Grade) / Borderline nuclear change in Endocervical cells.

We are very interested to know what users feel about this and how useful it would be to have reports broken down by these categories. In the meantime we will be talking closely to QA teams and key users.

We are waiting to see whether the National Office will change the definitions to bring them in line with each other.

It enables full patient histories to be accessed from within Labs and Clinics, follow up’s to be checked, failsafe routines to be run and so on.
